Lactose intolerance means your small intestine does not digest all the lactose—the natural sugar in milk—that you consume. The undigested lactose reaches the colon, where bacteria break it down and produce gas and fluid. This can cause bloating, abdominal pain, rumbling, gas, nausea or loose stools within a few hours of dairy. It is not the same as a milk allergy, and it does not always require giving up every dairy food. The useful next step is to track the dairy form and amount, rule out warning signs, and find your tolerance with professional help when symptoms persist.

That answer can feel awkward in an Indian kitchen where milk enters chai, dahi, paneer, kheer, mithai and packaged foods in very different amounts. But lactose tolerance is not a test of culture or willpower. Two people at the same table can respond differently, and the same person may tolerate a little milk in chai but not a large milkshake.

What is lactose intolerance?

Lactose intolerance is the occurrence of digestive symptoms after consuming lactose because the small intestine does not digest all of it. Lactose malabsorption describes the reduced digestion itself; it becomes lactose intolerance only when it also causes symptoms.

The enzyme lactase normally splits lactose into glucose and galactose so they can be absorbed. When lactase activity is lower than the lactose load, some lactose moves into the colon. Bacteria ferment it, creating gas, while undigested lactose can draw fluid into the bowel. That combination explains the familiar bloating, wind and diarrhoea better than the vague idea that milk is simply ‘heavy’. The most common symptoms of lactose intolerance include nausea, abdominal cramps, flatulence, diarrhoea, bloating, urgency with bowel movements, etc. Not everyone with low lactase activity has symptoms.

Why can lactose intolerance appear later in life?

Many people produce less lactase after infancy; this is called lactase non-persistence. The age at which it happens and the amount of lactose that remains comfortable vary. Symptoms may therefore become noticeable in later childhood, the teenage years or adulthood even when milk was previously part of daily life.

Lactose intolerance can also be secondary. A gut infection or another problem that affects the small-intestinal lining can temporarily or persistently reduce lactase activity. New symptoms after gastroenteritis may improve as the intestine recovers, while symptoms linked with coeliac disease, inflammatory bowel disease or another condition need the underlying cause assessed. Do not assume every new reaction is simply ageing.

How common is lactose intolerance in India?

There is no single Bible-verified percentage that can responsibly describe all Indians. Studies differ in population, region, age and whether they measure genes, lactose malabsorption on a test, or actual symptoms after dairy. Those are not interchangeable outcomes.

Lactase persistence does vary among populations and families, including within India, but a North-versus-South rule is too crude for an individual decision. A history of dairy farming also cannot tell you whether your own symptoms are caused by lactose. Your repeated food-and-symptom pattern is more useful than a regional label—and a clinician can test the diagnosis if the picture remains unclear.

Hives, itching, swelling of the lips or face, wheezing, throat tightness, breathing difficulty, dizziness or collapse are not ordinary lactose-intolerance symptoms. They can signal an allergic reaction. Use prescribed emergency medication if applicable and seek emergency care for breathing difficulty, throat or tongue swelling, fainting or rapidly worsening symptoms. Do not perform a home dairy challenge after a suspected allergic reaction.

The amount of milk matters

Lactose tolerance is often dose-dependent. A small amount of milk divided across the day may be easier to tolerate than a large glass at once. Water in chai reduces the concentration, but it does not remove lactose: the actual quantity and quality of milk used is what changes the lactose load.

Eating dairy with a meal may change the response

Some people tolerate a modest dairy portion better with food than on an empty stomach. The rest of the meal also matters. Ice cream, for example, contains lactose but may also be high in fat or include sweeteners and flavourings that affect symptoms. A reaction to one elaborate dessert does not prove that lactose alone was responsible. According to research, complete lactose elimination is rarely required, as most individuals tolerate up to 15 g of lactose daily, especially when consumed with other foods.

Dairy foods do not contain identical amounts of lactose

Fermentation and processing can reduce lactose, which is why some people find plain dahi or aged hard cheese easier than milk. This is not guaranteed. Homemade curd varies with the milk, culture, fermentation and portion, while paneer can retain different amounts of lactose depending on how it is made and drained. Chaas still contains lactose unless made from a lactose-free base.

Ghee is mostly milk fat and generally contains very little lactose, but ‘very little’ is not the same as a universal guarantee. More importantly, lactose content does not determine allergy safety: a person with milk-protein allergy should follow their allergy plan rather than assuming ghee or lactose-free dairy is safe.

Could the symptoms be caused by something else?

Bloating, gas, pain and loose stools are not unique to lactose intolerance. Irritable bowel syndrome, coeliac disease, inflammatory bowel disease, small-intestinal bacterial overgrowth and other digestive problems can overlap. A large, high-fat meal, sugar alcohols in a ‘sugar-free’ dessert, or another ingredient eaten with dairy may also be responsible.

Stress can make gut symptoms feel stronger or change bowel habits, but it does not prove that lactase production switched off for the day.

How do you find out if lactose is the issue?

Start with a seven-day pattern log

Record the dairy food, approximate portion, time, what else was in the meal and any symptoms over the next few hours. Note bloating, pain, gas, rumbling, nausea and stool changes. Also record symptom-free dairy exposures: the spoon of curd that caused no problem can be as informative as the milkshake that did.

If there has been no allergic reaction and no warning sign, a clinician or dietitian may suggest a time-limited reduction followed by a structured reintroduction. Change one variable at a time. A universal ghee-to-curd-to-chaas-to-paneer-to-milk ladder is not a validated self-test because the products, portions and lactose content vary.

What is a lactose intolerance test?

For a hydrogen breath test, a measured lactose drink is followed by breath samples and symptom recording over several hours. A rise in breath hydrogen together with symptoms can support the diagnosis. Testing is particularly useful when restriction has not given a clear answer or another condition may be mimicking lactose intolerance.

Stool acidity test: This test detects low stool pH due to fermentation of unabsorbed lactose into lactic acid.

Milk tolerance test: After administering 500 mL of milk, a rise in blood glucose < 9 mg/dL suggests lactose malabsorption.

Lactose tolerance test: This test involves ingestion of 50 g of lactose with serial glucose measurements at 0, 60, and 120 minutes. A rise of <20 mg/dL suggests lactose intolerance. Sensitivity is 75%; specificity is 96%. False-negative results may occur in patients with diabetes mellitus or small intestinal bacterial overgrowth. The results are also affected by delayed gastric emptying.

How to keep dairy without keeping the discomfort

The aim is the least restriction that keeps you comfortable. Depending on your pattern, that may mean a smaller portion, having it with a meal, choosing a tolerated fermented or lower-lactose food, or using lactose-reduced or lactose-free milk. Lactase tablets or drops help some people; ask a doctor or pharmacist how to use a product rather than assuming one dose fits every meal.

Check labels when small amounts trigger symptoms. Milk powder, whey, milk solids and curds can appear in bread, biscuits, instant soups, snack seasonings, protein powders and sweets. ‘Non-dairy’ wording does not always mean lactose-free, and ‘lactose-free’ does not mean free of milk protein.

Plain curd or fermented yoghurt can be part of a balanced Indian plate if you tolerate it, but fermentation does not turn every homemade product into a proven probiotic.

If you reduce dairy, replace the nutrition—not just the ingredient

Milk and dairy can contribute protein and calcium, so a broad exclusion deserves a plan. Plant-based milk alternatives, such as almond, coconut, and soy, are lactose-free options. Fortified plant beverages vary widely in calcium, protein, added sugar and other nutrients; check the label instead of treating almond, oat, coconut and soy drinks as nutritionally identical. A dietitian can help children, pregnant people, older adults or anyone with a very restricted diet avoid gaps.

A dairy-reduced Indian lunch could include roti, sabzi and Aashirvaad Organic Tur Dal as an everyday pulse option, with calcium planned through tolerated dairy or appropriate calcium-containing and fortified foods elsewhere. The dal supports meal variety; it is not a lactose-intolerance treatment or a one-for-one nutritional replacement for milk.

Try this: find the smallest useful change

After the seven-day log, choose one next step: reduce the portion of the clearest trigger, compare ordinary milk with a lactose-free version, or take the log to a clinician. Do not deliberately retest a food that caused hives, swelling, wheeze, faintness or another possible allergic reaction.
