class="wp-block-heading">Quick Answer

A food and symptom diary records what you eat and drink, when you have it, any symptoms and the context around them. It can help a doctor or dietitian explore possible patterns, but it cannot diagnose a food intolerance, allergy or digestive condition.

Seven days is a practical starting point, not a required duration. Choose ordinary days and keep your usual routine, including any existing medical dietary advice. Do not remove suspected foods or start a restrictive diet simply to test a theory during the week.

Write meals and drinks down soon afterwards, using portions such as two rotis, a katori of dal or a cup of chai. Include snacks, sauces, supplements and medicines. There is no need to count calories or weigh everything.

When symptoms occur, note what happened, when it began, how long it lasted and how strong it felt. Use the same rating throughout, and record symptom-free periods too. Sleep, stress, eating pace and bowel movements can provide useful context.

At the end of the week, look at comfortable days as well as difficult ones. A repeated association is a question for discussion, not proof that a food caused the problem. Take the record to your clinician rather than turning it into a banned-foods list.

Do not wait for seven days if warning signs appear. Blood in stools, unexplained weight loss, swallowing difficulty or persistent vomiting need prompt assessment. Sudden severe abdominal pain, vomiting blood or black, tarry stools need emergency care. Persistent symptoms deserve medical review.

Why keep a food and symptom diary?

Picture it: your stomach feels tight around four in the afternoon, but lunch was hours ago and there was a cup of chai in between. By night, three meals have run together and nobody, you included, can say which one did what. The diary exists to close that gap.

A food and symptom diary can help a clinician explore possible triggers. Its value is a clear record, rather than a memory that has already blurred.

Seven days is a workable first stretch. It is not a diagnostic threshold, and your doctor or dietitian may ask for a longer record or a different format.

Keep the week ordinary

Eat the way you normally eat, while continuing any existing medical dietary advice. A diary of a tidied-up week only describes the tidied-up week. If Sunday means puris and Thursday means a skipped lunch, let both appear.

This is not a calorie count or the first stage of a self-prescribed elimination plan. Do not remove suspected foods simply because they appeared beside a symptom. Equally, do not reintroduce foods you have been medically advised to avoid for the sake of completing the diary.

The aim is to observe your routine, not redesign it before anyone has read the notes.

Five things worth writing down

1. What you ate and drank, and how you ate it

“Dal chawal” is a start. “Moong dal khichdi, ghee, a spoon of mango pickle, finished in ten minutes at the desk” is a record.

Include the time, main ingredients and approximate portions. A katori, cup, spoonful or number of rotis is useful; there is no need to weigh everything. Include snacks, sauces, chai, coffee, alcohol and fizzy drinks where relevant.

Eating pace is useful context because eating or drinking quickly can increase swallowed air. Fizzy drinks can also contribute to gas symptoms.

2. What you felt, when it began and how strong it was

Give each symptom a start time, a description and, where possible, an end time. Note whether anything happened around the time it eased, without assuming that action caused the improvement.

You can use a personal scale from 1 to 10, with 1 meaning very mild and 10 meaning the strongest symptom you can imagine. Use it consistently. This is a way to describe your experience, not a clinical test or a reason to delay care.

Bloating is a feeling of fullness or tightness; visible abdominal swelling is worth recording separately. Burping, wind, chest burning, nausea and heaviness after meals can all go here.

Record comfortable periods too: “no symptoms this morning” is useful information. For bowel symptoms, note when you went, whether the stool was hard, soft or loose, and whether passing it was difficult. Cambridge University Hospitals’ diary guidance includes food, mood and bowel symptoms together.

3. Sleep

Write down roughly how long you slept and how rested you felt. This gives you something to compare across days, rather than assuming that food was the only thing that changed.

4. Stress

Low, medium or high is enough. If a particular event mattered, add a few words: an exam, travel, a difficult meeting or a deadline.

Stress can affect digestion, and people respond differently. Some notice bloating or constipation; others notice looser stools or less appetite. Recording stress does not prove that a symptom is caused by it.

5. Medicines and supplements

Include names and timings, even for occasional antacids, iron, calcium or vitamin D. Record the dose if you know it and note any recent changes.

Do not stop or alter a prescribed medicine to test a diary theory. Take the question to your clinician.

A plain notebook or phone note works. What matters is a record you can use consistently.

What one day of notes looks like

Invented example: Day 2, Tuesday. These entries illustrate the format; they are not evidence that any listed food or activity causes or treats symptoms.

Time

Food, drink or symptom

7:45 am

Two idlis, sambar, filter coffee with sugar. If a detail is missing, mark it as missing rather than guessing.

Why one week cannot name a culprit

Look again at that Tuesday. The gobi sabzi might seem an obvious suspect for the afternoon tightness. But the rushed breakfast, short night and deadline are all on the same page.

Bloating has several possible causes, including gas, constipation, food intolerance and irritable bowel syndrome. In some people with IBS, certain fermentable carbohydrates called FODMAPs can contribute to symptoms. These carbohydrates occur in many ordinary foods; a diary does not establish that they are your problem.

This is how a diary can become a banned-foods list: a dish appears twice beside a bad afternoon, and by day four it has left the kitchen.

Unnecessary exclusions can make a diet less varied. The ICMR-NIN My Plate for the Day policy brief describes sourcing nutrients from eight to ten food groups daily and emphasises variety within food groups. That does not mean every individual food is essential, but it is a reason to avoid removing foods without a clear plan.

So record first and conclude later. If one food keeps turning up, note it and take the question to a professional. Do not begin a low-FODMAP or other restrictive diet on the strength of the diary alone.

How to read your diary on Day Eight

Sit down with the pages and ask three questions:

When did each symptom begin and end? Compare it with the timing of earlier meals, drinks and medicines. Do not automatically assign it to the closest meal: timing alone cannot establish a cause.

What did the difficult days share besides food? Look at sleep, stress, eating pace, portions and changes in routine.

What did the comfortable days share? Good days are data too, and they are easy to overlook.

Underline what repeats. Cross nothing out. Take the notebook, phone note or photographs of the pages to your doctor or dietitian.

A pattern can guide questions and further assessment. It cannot confirm an intolerance, allergy or digestive diagnosis. If symptoms persist or keep returning, arrange an appointment rather than extending the diary indefinitely.

That includes the chest burning in the sample day: frequent or persistent reflux should be checked by a doctor.

When not to wait until day seven

The diary can be shown as it stands. Do not finish the week before seeking help.

Seek prompt medical assessment for blood in your stool, unexplained weight loss, difficulty swallowing, persistent vomiting or ongoing abdominal pain.

Get emergency medical help for sudden severe abdominal pain, vomiting blood or black, tarry stools.

Do not assume chest pain is reflux. Sudden chest pain that does not go away, especially with breathlessness, sweating or pain spreading to the arm or jaw, needs emergency help.

A symptom score is not a triage tool. A concerning symptom needs attention even if you have given it a low number.

Who should seek advice before starting?

If you have a current or past eating disorder, discuss this self-directed diary with your treating clinician or dietitian first. They can advise whether recording food is appropriate and how to do it.

If writing things down makes you anxious, encourages repeated checking or pushes you to restrict, stop and seek advice. The diary is optional; you can describe symptoms to a clinician without completing it.

For children, during pregnancy or for older adults with health needs affecting eating, seek individual guidance before making dietary changes.

What the diary may show about your plate

Your seven days can also show which food groups appear regularly. Keep this as a discussion for after the record, rather than a reason to change the week halfway through.

ICMR-NIN’s 2024 dietary guidance recommends at least half of cereals and other grains as whole or minimally polished grains. Its 2,000 kcal reference plate includes 400g vegetables and 100g fruit daily, alongside other food groups including pulses and milk or curd.

These are reference amounts, not a treatment plan for symptoms. Milk or curd may not suit everyone; discuss suitable alternatives if needed.

Look at how often your meals included whole grains, pulses, vegetables and fruit. If changes would help, agree them with your doctor or dietitian and make them gradually.

NIDDK advises increasing fibre a little at a time and drinking enough liquids to help it work. A sudden increase can cause gas or bloating. Different fibres behave differently, so “more fibre” is not a universal answer to digestive discomfort.

Whole-wheat atta and Khapli atta can both contribute fibre. If you use Aashirvaad Namma Chakki 100% Khapli Atta, use the current pack’s nutrition panel and the amount of flour used when estimating its contribution. Roti size varies, so three rotis are not a standard portion across households. Khapli is a way to vary whole-grain rotis, not a digestive shortcut.

Some conditions require a lower-fibre diet or individual fluid limits. Follow your existing medical advice rather than using the diary as a reason to override it.

Getting started

Keep the notebook or a phone note within reach and write tomorrow’s first meal down soon afterwards, while the details are clear. Add symptoms when they occur, even if they appear hours later.

A half-filled line still counts. A skipped day is simply a gap to mark.
