Whole grains, vegetables, fruit, legumes and nuts do much of the work. Its gut-health evidence is still developing, and following its name or proportions doesn’t guarantee a particular microbiome response. Nutrient needs, food access and affordability still matter.

Where This Diet Came From

Genuinely different from a lived culinary tradition, this one was designed as a global reference framework. The EAT–Lancet Commission, originally co-chaired by Walter Willett and Johan Rockström, published it in 2019 to address healthy diets from sustainable food systems for a world approaching 10 billion people by 2050.

The 2025 Commission updated that work, with greater attention to justice, local adaptation and the conditions needed to make healthy food accessible. It isn’t a framework frozen in 2019.

Its familiar features are roughly half a plate of fruit and vegetables, with whole grains, legumes, nuts and other protein foods making up much of the rest. Modest dairy, fish, eggs or poultry can fit; red meat and added sugar are limited. The plate picture is a guide, not a fixed meal prescription for every age, body or medical condition.

Is the Planetary Health Diet Actually a Health Diet, or Is It Really About the Environment?

It’s genuinely both. This one wasn’t built by observing a single culture that had eaten a certain way for generations. It was assembled from evidence about diet and health, then considered alongside the question of what the planet can sustainably produce.

That distinction is worth getting right. Human health wasn’t an afterthought or merely a constraint on an environmental goal. EAT describes the dietary pattern as based on human-health evidence, with environmental impacts assessed alongside it. The wider Commission asks how food systems can deliver both.

A global framework still has to work once real people try to live on it. That means looking at the food on the plate, but also who can buy it, what grows locally and whether it meets someone’s nutritional needs. Those are part of evaluating the idea, rather than awkward details to smooth over.

Okay, but Does It Actually Help Your Gut? Is There Real Research?

There’s a specific study worth being precise about, because the honest answer is more tentative than you might expect from how confidently this diet gets recommended.

In a 2023 descriptive study by Rehner and colleagues, researchers followed 41 healthy adults in Germany for 12 weeks. Sixteen switched to the Planetary Health Diet, while 16 omnivores and nine vegetarians or vegans continued their usual diets. People chose their dietary group; this wasn’t a randomised controlled trial.

The Planetary group showed trends towards increases in some bacteria, but the highlighted changes weren’t statistically significant. Overall microbial diversity remained relatively stable. That makes this an early finding worth following up, rather than firm evidence that the diet improves gut health.

The fermentation explanation is still useful. Non-digestible plant fibres can be fermented by colonic bacteria into short-chain fatty acids, or SCFAs, including acetate, propionate and butyrate. Butyrate helps fuel the cells lining the colon and supports gut-barrier function and immune regulation. That helps explain the interest in fibre-rich eating. It doesn’t mean this particular study proved stronger fermentation or a clinical digestive benefit.

Wait, So Is Going Fully Vegetarian Actually Better for Your Gut Than This Diet Specifically?

I wouldn’t take that verdict from this study. The vegetarian and vegan group had higher amounts of some bacteria, including at the beginning. But they were already eating that way, and they weren’t randomly assigned to it. The groups were also small and differed in their sex balance.

That’s a useful, humbling distinction: having more of a particular bacterium isn’t the same as having better gut health. This study doesn’t establish that vegetarian or vegan eating is superior, or that removing the remaining animal foods causes a better response.

It’s reasonable to think about how much plant fibre actually reaches the gut. But a vegetarian label doesn’t tell us that on its own. A varied diet built around dals, whole grains, vegetables and fruit is a different proposition from one dominated by refined starches and sweets. I’d look at the food pattern before declaring a winning label.

I’ve Heard People Criticise This Diet for Making It Hard to Get Enough Nutrients. Is That Fair?

It’s a real, published concern, not just internet pushback. A 2023 analysis in The Lancet Planetary Health estimated shortfalls in vitamin B12, calcium, iron and zinc in the original reference diet, accounting for factors such as nutrient bioavailability. The authors proposed changes to improve adequacy without fortification or supplementation.

That’s a scientific debate about a model, rather than proof that everyone following a plant-rich diet will become deficient. The food choices, quantities, life stage and assumptions used in the analysis matter. It also shouldn’t be treated as an assessment of every adaptation of the framework, including its subsequent update.

The practical point is to plan the nutrients as well as the proportions. If you substantially reduce animal-source foods, consider what will supply protein, vitamin B12, iron, zinc and calcium. A fully vegan version needs a reliable B12 source through appropriately fortified foods or supplementation. A dietitian can help work out what fits your needs, rather than leaving that to a plate diagram.

Is This Realistic or Affordable for Most People, Especially Outside Wealthy Countries?

That’s a central criticism, and familiar ingredients don’t make it disappear. An international affordability analysis found that the reference diet could exceed the entire per-person household income of many people. This was an estimate using historical prices and incomes, not a current shopping bill, but the access problem it identified was substantial. Research in rural India has examined that gap locally too.

For an Indian reader, though, there are familiar places to start. A plate built around dal, seasonal vegetables and whole grains can apply the framework’s plant-forward logic without importing a new pantry. That doesn’t mean every traditional Indian plate already matches its proportions, or that the full pattern is affordable for every household.

Whole-wheat atta, including whole-grain Khapli options such as Aashirvaad Chakki 100% Khapli Atta, can contribute fibre and plant protein. Khapli is emmer wheat, also known as Triticum dicoccum, rather than a millet. Wheat fibre includes arabinoxylans, some of which can be fermented by gut microbes. It’s one grain choice, not a requirement for the framework or a guarantee of a particular microbial effect.

Paired with dal and local produce, whole grains offer a practical starting point. Whether a meal meets wider environmental goals also depends on how food is produced and wasted; one branded flour can’t establish that by itself. And a premium grain isn’t necessary: ordinary whole-wheat atta and locally available millets can fit too.

If I Only Take One Thing From This Diet, What Should It Be?

Lean towards more plant variety and fewer ultra-processed foods that crowd out balanced meals. That’s a useful direction without treating the exact food-group ratios as a personalised nutritional prescription.

I’d keep the practical question close: what can you regularly cook, afford and enjoy? Dal, seasonal vegetables, whole grains, fruit, nuts or seeds give you plenty to work with. If you’re increasing fibre, do it gradually and keep fluids adequate. Our guide to increasing fibre in an Indian diet turns that into everyday food choices. Follow any specific dietary instructions you’ve been given for a medical condition.

What a Day on This Diet Might Look Like

Not a prescription, just an illustration of the broad pattern, built around an Indian plate. Portions and nutrient planning need to suit your needs.

Breakfast: seasonal fruit, a whole-grain porridge made with oats or a millet-based option, and a few nuts.

Lunch: a large serving of vegetable sabzi, dal, brown rice or rotis made from whole-grain atta such as Aashirvaad Chakki 100% Khapli Atta, and a small serving of curd. Millet rotis are another option; Khapli rotis are wheat rotis.

Dinner: a legume curry or a modest portion of fish or egg, sautéed seasonal greens, and whole-grain Khapli rotis or another whole-grain staple.

Snacks: fruit, nuts or seeds.

Red meat and added sugar stay limited under this framework. Modest fish, poultry, eggs or dairy can fit according to preferences and nutritional needs; the pattern doesn’t require everyone to eat every animal food. Vegetarian adaptations need suitable replacements rather than simply smaller meals. Our tofu and paneer comparison explains two familiar protein options.

Where It May Fit Easily, and Where It Takes More Planning

Might Genuinely Suit You

Might Need More Planning

You already eat plant-forward meals with modest animal foods.

Red meat or dairy is central to your meals and you aren’t looking to change that.

You care about environmental impact alongside personal health.

You need a therapeutic diet tailored to a health condition rather than a general framework.

You’re comfortable adapting a global reference pattern to local foods.

You want an exact menu or expect a predictable microbiome change from the diet’s name.

You can plan nutrient sources when reducing animal foods.

Cost, food availability or access to personalised guidance makes a major change difficult.

Key Takeaways

The Planetary Health Diet brings human health and sustainable food systems together.
