phenomenon studied in certain diseases. But it is not a standalone diagnosis, and commercial tests, supplements and restrictive diets cannot reliably diagnose or ‘heal’ it.

Expert reviewed by Qurath Ain, Certified Nutritionist and Clinical Dietitian

‘Leaky gut’ is not a recognised standalone medical diagnosis. The term loosely refers to increased intestinal permeability: a measurable change in how the intestinal barrier allows substances to pass through it. Barrier changes occur in some digestive diseases and remain an active area of research. What has not been established is the much broader wellness claim that an undiagnosed leak causes bloating, fatigue, eczema, anxiety, weight gain and many other problems—or that a stool test, supplement stack or restrictive diet can identify and ‘seal’ it. Persistent symptoms deserve a proper assessment for their actual cause.

That distinction is not a dismissal of how unwell someone may feel. Bloating, pain, diarrhoea, constipation and fatigue are real, but they are non-specific. Giving them one catch-all label can delay investigation for coeliac disease, inflammatory bowel disease, an intolerance, medication effects or another explanation.

What does ‘leaky gut’ mean?

Intestinal permeability describes how substances move across the lining of the intestine. The intestinal barrier is naturally selective rather than completely sealed: it absorbs water and nutrients while limiting the passage of microbes and other material from the gut.

The barrier is more than a single row of cells. It includes mucus, epithelial cells, immune defences and junctions between cells, with different routes for different molecules. When researchers describe increased permeability, they mean that passage through one or more of these routes has changed. It is not quite the dramatic picture of large holes opening and undigested food pouring directly into the bloodstream.

When the microbial balance in your gut is right, your whole body functions the way it’s supposed to. But when that balance gets out of whack — say because of chronic stress, chronic constipation, exposure to environmental toxins like pesticides, eating a poor diet, or taking an antibiotic that wipes out a lot of those microbes — the “bad” bacteria cut holes in the fence, and some of them, along with food particles and toxins, leak into the bloodstream. When your immune system sees organisms where they don’t belong, it attacks, causing irritation and inflammation.

Is leaky gut a real medical condition?

Increased intestinal permeability is a genuine biological finding. Researchers study it in laboratory models and in people with conditions including coeliac disease and inflammatory bowel disease. Changes have also been reported in some other disorders, but results depend on the condition, part of the gut and measurement method. The leakage in leaky gut may be responsible for a huge variety of health issues, ranging from minor (bloating, cramps, fatigue, food allergies and sensitivities, gas, and headaches) to “bigger things”: autoimmune conditions, depression and other mood disorders, diabetes, inflammatory bowel disease, and multiple sclerosis.

‘Leaky gut syndrome’ is different. It proposes that increased permeability is an independent, hidden disorder responsible for a long list of digestive and whole-body symptoms. That syndrome is not an accepted routine diagnosis, and research associations do not show that permeability was the original cause of a person’s illness. In some diseases it may be a consequence, contributor or marker; the direction can be difficult to untangle.

Leaky gut claims: what the evidence does and does not show

Popular claim

What the evidence supports

What it does not prove

‘Leaky gut’ is a diagnosis

Increased permeability can be measured in research and occurs in some diseases

A standalone syndrome explains a wide range of common symptoms

Bloating or fatigue proves it

These symptoms are real and deserve a cause-based assessment

That symptoms identify a change in the intestinal barrier

A zonulin test confirms it

Biomarkers and permeability methods remain research tools in many contexts

That one commercial blood or stool score is a validated diagnosis

Dysbiosis opens the gut

Microbes, the barrier, medicines, diet and disease can interact

That one stool pattern caused an individual’s symptoms

A protocol can seal the gut

Underlying diagnosed conditions may require specific treatment

That a general supplement stack or restrictive diet repairs everyone’s barrier

More fibre always repairs it

Fibre feeds gut bacteria and supports general digestive health

That high fibre treats permeability or suits every digestive condition

What are the symptoms of leaky gut?

There is no validated symptom checklist that can diagnose increased intestinal permeability. Abdominal pain, bloating, gas and altered bowel habits occur in many digestive conditions and sometimes without disease. Fatigue, headaches, skin problems, joint pain, ‘brain fog’ and food sensitivities are even less specific.

A long symptom list can feel convincing because many people recognise something on it. It still cannot reveal what is happening at the intestinal barrier. The useful question is not ‘Which leaky-gut symptom do I have?’ but ‘What established causes fit this pattern, and does it need assessment?’

What can change intestinal permeability?

Clearer examples involve an underlying disease or substantial injury to the intestinal lining. Coeliac disease and inflammatory bowel disease can affect barrier function. Chemotherapy, radiation and some infections may injure the mucosa. Frequent or prolonged use of non-steroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, can also affect the gastrointestinal tract; chronic heavy alcohol exposure is another recognised concern.

Context matters. This does not mean one painkiller or antibiotic tablet or one rich meal has created a chronic ‘leak’. Do not stop a prescribed medicine because of an online gut claim. If you use an NSAID often, have digestive symptoms or are unsure about dose and duration, discuss it with the prescriber or pharmacist.

Diet, stress, sleep and the microbiome are also studied in barrier research, but consumer claims often overstate the human evidence. Stress can speed up or slow the gut and heighten pain or bloating; that does not let symptoms prove that tight junctions have opened. A diet pattern can influence gut microbes and health without becoming a diagnostic test for intestinal permeability.

Can the gut microbiome cause a leaky gut?

Microbes and the intestinal barrier interact, but ‘good bacteria versus bad bacteria’ is too crude. Different microbial communities can produce compounds that interact with mucus, epithelial cells and immune activity. Dysbiosis of the gut microbiota can affect motility and gut permeability, which may cause a leaky gut.. Disease, diet, medicines and inflammation can also change the microbiome, making cause and effect difficult to separate.

The word dysbiosis describes an altered microbial community; it is not one standard pattern and is not a routine diagnosis. A commercial stool report cannot show that an individual’s microbiome opened the gut barrier, nor can a diversity score establish which supplement will repair it.

Can a test diagnose leaky gut?

There is no validated protocol or diagnostic tool to identify or diagnose leaky gut. Researchers can investigate permeability with methods such as giving differently sized sugars and measuring how much appears in urine. Other approaches examine tissue or use specialised imaging and laboratory techniques. Each method captures particular routes or regions, and results are sensitive to protocol and interpretation.

That is not the same as having one standard clinical test for ‘leaky gut syndrome’. Commercial blood or stool tests sometimes rely on zonulin or combine several markers into a permeability score. Research has raised serious concerns about what some widely used zonulin assays actually detect and how well their results correspond to intestinal permeability. A consumer result should not be used alone to diagnose symptoms, prescribe an elimination diet or sell a supplement plan.

Do probiotics, glutamine or special diets heal leaky gut?

No probiotic, collagen powder, glutamine supplement, herbal cleanse or packaged protocol has been shown to ‘seal the gut’ for everyone. Probiotic effects are strain-, dose- and condition-specific. A study of one formulation or one diagnosed group cannot validate every product bearing the same broad label.

The same caution applies to restrictive diets. Removing gluten is medically necessary for coeliac disease, but starting a gluten-free diet before testing can complicate diagnosis. A low-FODMAP diet may be used for IBS with professional guidance; it is not a general leaky-gut repair programme. Cutting multiple foods without a clear reason can narrow the diet, raise costs and make eating socially difficult.

Plain curd or another tolerated fermented food can sit within an ordinary Indian meal, but it is not a cure.

What can you do instead of a ‘gut-healing’ protocol?

Start with the problem you can observe. If it is bloating, note when it occurs, meal size, bowel pattern and possible triggers. If it is diarrhoea, pain or weight change, take the pattern to a clinician rather than treating a presumed barrier defect. Review frequent over-the-counter painkiller use with a doctor or pharmacist, and avoid tests or packages that make one result the explanation for everything.

For general eating, aim for variety rather than a repair ingredient: vegetables, fruit, dals or other pulses, nuts or seeds as appropriate, and grain foods you tolerate. Increase fibre gradually and drink enough fluid. A plate of sabzi, dal, curd and rotis made with Aashirvaad Multigrain Atta can be one familiar option if it suits you; the atta is not a treatment for intestinal permeability, and a higher-fibre pattern is not appropriate for every digestive condition.

Remove foods that create problems, for instance, gluten, sugar, and dairy.

Replace with foods less likely to irritate the gut: Fermented foods such as sauerkraut, kimchi, yoghurt, kefir, and pickles are healing foods.

Under medical supervision, consume supplements: L-glutamine (to heal the intestinal lining), vitamin D, zinc, and omega-3 fatty acids (such as fish oil).

Repopulate your good gut bacteria: probiotics and prebiotics can be helpful.

Keep yourself well hydrated: 8-10 glasses of water.

If you have a stricture, gastroparesis, an inflammatory bowel disease flare, recent gastrointestinal surgery or another condition requiring fibre restriction, follow your clinical team’s advice.
